In Baldur’s Gate 3, Soul Coins are story items that you’ll find during your travels through the world of Faerûn. Furthermore, Soul Coins are rare items to come by, and the description states that they can power Infernal Engines, which should make them quite useful.

How To Use Soul Coins in Baldur’s Gate III
In Baldur’s Gate III, Karlach is the only companion who can use Soul Coins after you’ve recruited her. As Karlach’s heart is an Infernal Engine, she can use Soul Coins to receive a passive buff.
Consuming a Soul Coin by Karlach grants her the Infernal Fury, a passive buff that increases her attack power and grants fire damage to Melee Attacks when her HP drops below 25%.
Transfer the Soul Coins to Karlach’s inventory, right-click them, and select Consume (Karlach). Each coin can be used three times before disappearing. Keep in mind that this buff disappears if you rest.
Soul Coin #1: Dank Crypt – Inside a Sarcophagus
The first BG3 Soul Coin can be found in the trapped sarcophagus room of the Dank Crypt. After the wreck with the Nautiloid Ship, open the Ancient Door near the Overgrown Ruins to enter the Dank Crypt and reach the trapped sarcophagus.
In the same room, you’ll find the first Soul Coin alongside a Rapier and a Skill in one of the smaller sarcophaguses.
Soul Coin #2: Dank Crypt – Inside a Sarcophagus
The next Soul Coin can be found in a side room of the Dank Crypt behind the locked Heavy Oak Doors. After you collect the Engraved Key from the trapped sarcophagus, head north through the crypt and open the Heavy Oak Doors.
To the left, open the door to a side room with several broken sarcophaguses and only one still intact. Check the intact sarcophagus to collect the second Soul Coin in BG3.
Soul Coin #3: Dank Crypt – Inside Wither’s Richly Adorned Sarcophagus
The final Soul Coin in the Dank Crypt can be found inside Wither’s sarcophagus in the secret room behind the statue. Wither’s is the only NPC available in the game that will allow you to respec your character and companions in exchange for gold.
Leave the side room and go northwest of the statue, then Press the button to open his chamber. After pushing the button, defeat the five undead enemies and return to his sarcophagus. Have a quick chat with Withers to recruit him, then open his sarcophagus to find the third Soul Coin alongside many valuable items.
Soul Coin #4: Druid Grove – On Nadira
The fourth Soul Coin can be found in the possession of an NPC called Nadira at the top of a hill in Emerald Grove. After the battle at the gates against the goblins, go through the wooden gate and climb the ladder on the right to get to the top of the gate.
Then, take the path up leading to the east to find Nadira using a Telescope. As soon as you get close to her, she will be ambushed by a Bugbear Assassin. Defeat the Bugbear Assassin and use the [DECEPTION] dialogue option to get the fourth Soul Coin from her.
